You will need your email address and password you obtained when signing up for Entry Eeze. Be sure you are on the "Manage My Family" tab. Select the desired skater and then “Get a permission form” button. You must enable pop-ups BEFORE clicking on the button; if you enable afterwards, you may need to click on the button AGAIN.
